Preventing deconditioning in patients with heart failure
We are developing an ambulation and education program facilitated by a mobility technician and wearable sensors to promote early ambulation in patients admitted with congestive heart failure.
STORY
Heart failure (HF) patients become deconditioned while inpatient. Impaired stamina, coordination, and strength can lead to post hospital syndrome, longer length of stay and readmissions.
SOLUTION
We are developing an ambulation and education program, facilitated by a mobility technician. We are recording patient activity data EHR flow sheets, text messaging, and patient portal.
RESULTS
We have developed a program to help mobilize patients with heart failure during the inpatient visit. The data obtained through the EHR and activity sensors will be used to create tailored therapeutic pathways for each individual.
